Crossover Community Impact (CCI)
Clinical operations of CCI located at THD North Regional Health and Wellness Center
• 6,937 patient visits

Partnership with Access Medical and Mobile COVID Testing LLC
To provide free COVID testing to uninsured residents in Tulsa County
• Provides testing access to people who are mobility impaired or lack transportation
• 6,852 free COVID tests were provided to uninsured residents of Tulsa County

Partnership with Uma Center of Tulsa
To provide health education and vaccination resources to the Hispanic Community in Tulsa County
Uma Center of Tulsa held seven health resource events during the cold and flu season of winter 2022.
• 493 adults and children received their vaccinations at clinics hosted by Uma Tulsa
• 112 health screenings were conducted and 130 COVID vaccinations were given to 54 children
Be Well Community Development Corporation
The funding from CDC Health Equity Grant helps Tulsa Health Department support the operations of Be Well Community Development Corporation and the expansion of the THD Health Equity Office.

Tulsa Interpretation and Translation Services
Tulsa Health Department funds YWCA Tulsa Interpretation and Translation Services and Training Classes. YWCA Tulsa provides professional interpretation and translation services to ensure all Tulsans have access to services across the city.
• Current languages include Arabic, Burmese, Dari, French, Hmong, Mandarin, Pashto, Russian, Spanish, and Zomi
• Services include but are not limited to community, medical, school and legal purposes
• YWCA Tulsa responded to 85 interpretation requests in just one month in FY23
• YWCA Tulsa partners with several local organizations that serve clients with a variety of needs including persons overcoming substance abuse or escaping domestic violence to provide general and medical translation services

Blood At The Roots Conference
Issues of Health Equity conference held on April 13 – 14, 2023 to provide a comprehensive exploration of the pressing concerns surrounding health equity
• 250 attendees
• Five impactful keynote speakers and 50 breakout session presenters represented by various organizations 
• Overarching topics included the root causes of poor health outcomes for BIPOC communities, the racial wealth gap in education, community initiatives for health equity, and the unjust criminal justice system
• Be Well CDC, in collaboration with the Tulsa Birth Equity Initiative, organized a thought-provoking pre-conference event at the Circle Cinema on April 12, 2023. The screening of the documentary “Birthing Justice” was accompanied by an insightful panel discussion featuring accomplished doulas

Back-to-School Bash
Held on Aug 12, 2022, students received back-to-school supplies courtesy of Be Well Community Development Corporation and other generous sponsors.

• 797 community members from 303 families participated in this free event

• Over 400 backpacks, tons of school supplies, and personal hygiene care products were given out
• Access to books, treats, and various resources

• Preventive healthcare services crucial for school readiness were provided. Over 215 immunizations were given

Joyful Movement North Tulsa
Initiative to improve mental health and physical activity among north Tulsa residents, primarily living in zip code 74126
• Held in partnership with North Tulsa changemakers such as Jane A. Malone Center at Chamberlain Park
• 98 classes which consist of Yoga, Barre, Belly Dancing and Line Dancing were conducted in FY2022-23
• Summer Kick-Off Event on Jun 8, 2023. Launch of Couch to 5k challenge. 61 women participated in 5k events throughout summer

North Tulsa Forward Program
Housing grant launched in July 2022 to help with the purchase of homes in North Tulsa
• Four in-person financial literacy and homebuyer classes
•$10,000 housing grant was given to 14 participants
• Over 201 applicants received
• 54 graduated from Homebuyer & Financial Literacy courses
• 72 completed our online financial literacy and homebuyer training courses

Addressing Maternal Child Heath Through Awareness (AMEA)
Initiative designed to address maternal child health equity for pregnant and postpartum women in North Tulsa. The initiative has successfully completed the following goals:
• Convene stakeholders to understand systemic issues in maternal care delivery in Tulsa District 1
• Conduct data collection and analysis of expectant mothers and providers in Tulsa District 1 to build on existing data regarding maternal health practices and outcomes with particular emphasis on minority women
• Organize and conduct a learning laboratory to operationalize recommendations for improving maternal health outcomes
AMEA partnered with Haruv USA to conduct a training and simulation workshop ‘Building Trust To Improve Black Maternal Health in Tulsa” on April 21, 2023. Seventeen University of Tulsa nursing students and other community professionals attended the workshop. The attendees received a first-class learning experience from a panel of lived experience participants who shared their birthing stories and a simulated learning workshop.

Center for Community Health (CCH)

• Added another Community Health Worker (CHW)
• CCH services are now accessible at Saint Francis South and will include Laureate Psychiatric Clinic and Hospital soon
• Partner with the Oklahoma State Department of Health and the Oklahoma Healthcare authority to advocate for CHW Medicaid Reimbursement. We hope to see this crucial support mechanism expanded statewide
• Two of our dedicated team members have been awarded scholarships to attend the 2024 National Association of Community Health Workers Unity Conference and Annual Meeting. These scholarships were made possible through the Oklahoma Public Health Training Center CHW Program within the Hudson College of Public Health
Patient/Client Testimonials
One patient attributed their initial journey to sobriety to the care and listening provided by a Community Health Worker. The patient was struggling with self-doubt and addiction. The CHW connected with the medical team for evaluation and treatment when the patient expressed mental health concerns. Despite setbacks and relapses, the CHW worked on building the patient's self-efficacy and confidence in their own care. With the CHW’s support, the patient began scheduling transportation to their dialysis appointments and the patient’s repeat hospital visits decreased. Upon completing the Center for Community Health program, the patient proudly announced their 20 days of sobriety and thanked the CHW for their support.
In August, a CHW met the patient with compassion and empathy, and educated them about transportation services and the free summer bus program. Recognizing the patient’s other challenges, the CHW also connected them with local resources to acquire much needed furniture, and was able to provide them with clothing and shoes before they discharged. The CHW went a step further by assisting the patient in scheduling a follow-up appointment with their primary care physician. The patient expressed their heartfelt gratitude upon completion of the program, highlighting the transformative impact a dedicated CHW can have on a patient’s well-being.
Community Health Worker at Ascension St. John
A grant proposal was submitted to the Ascension St. John Community Health Equity Catalyst Strategy (CHECS) to launch an intervention focused Community Health Worker at Ascension St. John (ASJ). CHECS awarded THD $172,000 to fund the new program at ASJ including the salary of one program manager and one community health worker. The program is set to begin in the next fiscal year.

Child Guidance
Our Audiology program is one of two programs that provide Durable Medical Equipment (DME) services for Medicaid clients (hearing aids) for the entire east side of Oklahoma. A total of 120 hearing aids were dispensed over the fiscal year.
